Welcome to AccuGen Diagnostics, LLC (“AccuGen,” “we,” “our,” or “us”). By scheduling or receiving services from AccuGen, you (“Client,” “Patient,” or “you”) agree to the following Terms and Conditions. Please read carefully.
1. Scope of Services

AccuGen Diagnostics, LLC provides professional specimen collection, processing, handling, and transportation services.

AccuGen does not provide medical advice, diagnosis, or treatment. All medical questions must be directed to your licensed healthcare provider.

Medicare recipients must provide a valid physician’s order prior to specimen collection in order for the appointment to be billed properly and to ensure proper collection and avoid delays.

A valid laboratory script (lab order) must be submitted before the scheduled appointment to ensure proper collection and avoid delays.
2. Laboratory Deliveries & Courier Services

AccuGen ensures timely delivery of collected specimens to the appropriate laboratory.

AccuGen is not responsible for delays caused by third‑party couriers (e.g., FedEx), weather conditions, or unforeseen issues occurring after the specimen has been delivered or picked up.
3. Laboratory Errors

AccuGen is not responsible for laboratory processing errors.

If a laboratory error occurs, AccuGen will recollect the specimen at a discounted rate.

If an error is made by AccuGen during collection, handling, or transport, we will recollect the specimen at no additional charge.
4. Billing & Payment

Mobile appointments require full payment in advance to secure the appointment, protect our travel time, and ensure proper route planning.

Unpaid mobile appointments may be canceled.

AccuGen partners with laboratories, physician offices, and healthcare facilities as an independent contractor.
5. Booking, Cancellations & No‑Shows

Cancellations made 24 hours or more before the appointment are eligible for a refund (minus any applicable processing fees).

Cancellations made less than 24 hours before the appointment incur a $25 cancellation fee.
-
Same‑day cancellations are non‑refundable.
-
No‑shows may result in loss of future booking privileges.
-
Once a phlebotomist is en route, the appointment is considered active and is non‑refundable.
6. Client Responsibilities
-
Medicare clients must provide valid physician orders prior to collection.
-
All clients must submit required forms before the appointment to ensure a smooth visit.
-
Clients are responsible for providing accurate personal, insurance, and laboratory information at the time of service.
7. Gender Reveal Testing Policy
AccuGen Diagnostics partners with Peekaboo for early gender reveal testing. AccuGen’s role is limited strictly to the collection and handling of the blood specimen.

AccuGen is not responsible for the accuracy, interpretation, or outcome of Peekaboo laboratory reports.

All gender determination results are generated solely by the Peekaboo laboratory.

AccuGen does not guarantee or verify the accuracy of gender reveal results and is not liable for any decisions made based on those results.

Any questions regarding test accuracy, report findings, or laboratory methodology must be directed to Peekaboo.
8. DNA Testing Conduct Policy
To protect staff safety and maintain chain‑of‑custody integrity:

Arguing, fighting, hostility, or disruptive behavior of any kind is strictly prohibited during DNA testing appointments.

If such behavior occurs, the appointment will be immediately terminated with no refund, and the client may be prohibited from booking future services.

All parties must remain calm, cooperative, and respectful throughout the appointment.
9. Drug Testing Conduct & Adulteration Policy
AccuGen Diagnostics follows strict chain‑of‑custody standards for all drug and alcohol testing. To protect the integrity of the collection process:

Any attempt to tamper with, substitute, dilute, or adulterate a specimen will result in immediate termination of the appointment.

The test will be documented and reported as “Refusal to Test / Adulterated” in accordance with industry standards.

No refunds will be issued for appointments cancelled due to suspected tampering, non‑compliance, or refusal to cooperate.

Hostile, argumentative, or disruptive behavior during a drug testing appointment may also result in immediate cancellation with no refund and may prevent future bookings.
10. Limitation of Liability
AccuGen’s responsibility is limited strictly to the professional collection, processing, handling, and transportation of specimens.
AccuGen is not liable for:

Laboratory errors

Courier delays
-
Medical outcomes or decisions based on laboratory results
11. Amendments
AccuGen reserves the right to update or modify these Terms & Conditions at any time. Changes become effective immediately upon posting on our website or providing notice to clients.
12. Patient Privacy
AccuGen Diagnostics, LLC complies with all HIPAA regulations.
We protect all client information and do not disclose personal or health information to unauthorized individuals under any circumstances.
